GETTING STARTED

• Identify patients with neurological symptoms.
• Fill out the insurance form for each candidate and fax to 800-265-2544. In most cases, you will receive the Insurance Status Notification within 48 hours.
• For workman’s compensation patients, please fax the completed Medical Necessity Form with the Insurance Form.
• After receiving the faxed Insurance Status Notification, call 800-998-9777 to schedule a testing date.
• You must fill out the Report of Medical Necessity form for each patient tested. Please give copies to the technician on the day of testing. Keep the original for your files.
• Please complete the scheduling forms, taking notice to the required testing times, and give it to the technician on the day of testing.
• You will receive the results from our Board Certified Neurologist within 5 working days.
